The following forms are available from our web site to help you save time at your next visit. Please take a moment to fill out the appropriate form, and then either e-mail it to petvet@intermountainvet.com or bring it with you to your appointment.
When you come into the clinic for your appointment, you will be asked to electonically sign the forms that require a signature.

New Client Information Form
Complete this form if it is your first visit to Intermountain Pet Hospital, and then e-mail the form to petvet@intermountainvet.com.
Records Transfer Form
If you are bringing your pet to us from another clinic, please send them this completed form so they can fax us a copy of your pet's medical history.
Canine Risk Factors Form
Please complete this form in order to help us determine the correct preventative care protocol for your dog. When you have finished filling out the form, please e-mail it to petvet@intermountainvet.com before you bring your dog in to the clinic.
Early Drop-Off Treatment Form
In order to accommodate your work schedule and your pet's health needs, the clinic accepts patients that are left with us ("dropped off"). PLEASE CALL FIRST whenever possible. Complete this form, and then leave it with the receptionist or nurse at the time you leave your pet.
Prescription Refill Request Form
Complete this form if you need to refill a prescription for your pet. Please e-mail the completed form to petvet@intermountainvet.com and allow us 24 hours to fill the prescription.

Surgical Authorization Form
Complete this form if your pet is scheduled for surgery. Please e-mail the completed form to petvet@intermountainvet.com before you bring your pet in for the surgery appointment.
Oral Assessment, Treatment, and Prevention (ATP) Authorization Form
An Oral ATP is a very comprehensive oral examination that includes radiographs, a professional assessment of the exam results, treatment of problems found, and a specific plan for prevention of further problems.
If your pet is scheduled for Oral ATP, please complete this form, and then e-mail it to petvet@intermountainvet.com before you bring your pet in for the appointment.
Surgery with Oral ATP Authorization Form
If your pet is scheduled for surgery that also includes Oral ATP, please complete this form, and then e-mail it to petvet@intermountainvet.com before you bring your pet in for the surgery appointment.
Spay Surgical Authorization Form
Complete this form if your pet is scheduled for a Spay surgery. Please e-mail the completed form to petvet@intermountainvet.com before you bring your pet in for the surgery appointment.
Neuter Surgical Authorization Form
